Connecticut Home Care Programs offer an alternative to nursing home placement for Medicaid-eligible adults who are age 65+, age 18-64 with a disability or acquired brain injury. For older adults, there is also a state-funded program that allows residents to spend down assets until they are eligible for Medicaid. Care Managers work with the individuals to build a care plan that ensures safety and quality of life based on the individual’s preferences. Personal care, companionship, housekeeping, adult day, nursing and meal services are typically available as part of the plan.
