Callers in need of shelter must call 2-1-1 for a referral.Provides permanent housing assistance and support services to homeless individuals and families where a member has been diagnosed with a disability (such as mental illness, AIDS/HIV or substance abuse). Remove this sentence Program provides supportive housing for a five-year period. Support services include case management and referrals to other health and social service providers. Housing is in the Greater Hartford Area.
