Call to request a transportation packet. 48-hour advance notice required. Annual donation, or per ride fee donation is suggested. Ellington Medical Transportation service provides transportation to non-emergency medical appointments.

Application Process
Complete the Annual Registration and Release Form located online at: https://seniorcenter.ellington-ct.gov/registration-forms-payment/forms. If individuals need assistance, call the Senior Center at (860) 870-3133; Once submitted, a Transportation Packet will be mailed to individuals; Return the required Form 5 using a self-addressed, stamped, envelope provided to you to the Ellington Senior Center, 55 Main Street, PO Box 187, Ellington, CT 06029; Once received the form individuals will be notified via telephone and may begin scheduling rides; All Appointments are made with the secretary, not drivers. At least 48 hours advance notice is required.
